Is G-POEM for gastroparesis decided only by a gastric emptying test?
Gastroparesis requires both compatible symptoms and objective evidence of delayed gastric emptying. Nausea, vomiting, early satiety and post-meal fullness can also occur in other disorders.

G-POEM targets the pyloric muscle through an endoscopic tunnel. It should not be expected to work equally in every patient with gastroparesis and is generally considered in selected patients with persistent symptoms despite appropriate medical and nutritional management.

Mechanical obstruction should be excluded and diabetes control, medicines, nutritional status and previous treatments reviewed before intervention. In some centres, tests of pyloric physiology can add useful information.

After treatment, outcomes are assessed using symptoms, ability to eat, weight and quality of life—not only a change in gastric emptying measurements.
