Early care after POEM supports safe healing of the mucosal entry and muscle-cutting area. Depending on the centre’s protocol, liquids and then soft foods are introduced gradually. The treating team’s written plan is more important than generic internet diets.

Mild pressure, gas and discomfort may occur, but severe increasing pain, fever, breathing difficulty, repeated vomiting or inability to drink needs urgent review.

Long-term follow-up is not limited to asking whether swallowing improved. Reflux can develop after POEM, so symptoms may need to be considered alongside endoscopy or reflux testing when appropriate. Absence of heartburn does not always exclude reflux.

Achalasia is a chronic motility disorder, so weight, recurrent swallowing problems, nutrition and reflux remain important even after an effective procedure.
